Proposed Analysis: Cerebral small vessel disease (CSVD), a common cause of stroke and vascular dementia, manifests as white matter hyperintensities (WMHs), lacunas, microbleeds, and enlarged perivascular spaces (EPVS) on brain imaging. Several mechanisms of CSVD have been identified, including blood-brain barrier dysfunction, vessel stiffening, cerebral blood flow decreasing, interstitial lfluid (ISF) drainage dysfunction, and inflammation. However, the exact pathogenesis is still poorly understood and the intervention is lacked. Recent evidences showed the existence of a central nervous system ‘waste clearance’ system, defined as glymphatic system. Glymphatic dysfunction have been described in several neurodegenerative conditions, including Alzheimer’s and Parkinson’s disease. Glymphatic function has not been thoroughly explored in cerebral small vessel disease(CSVD), where neurodegenerative processes are intermingled with inflammatory processes. In 2017, Taoka et. al. derived a measure of glymphatic flux called “diffusion tensor imaging-analysis along the perivascular space” (DTI-ALPS) index and found that its alterations correlated with Mini-Mental State Examination (MMSE) scores in patients with AD and associated conditions and were thought to reflect glymphatic dysfunction. DTI-ALPS offers an opportunity for the noninvasive investigation of the human glymphatic system and thus we took advantage of this method to evaluate the glymphatic system in patients with CSVD. We aimed to establish associations between glymphatic function and, neuropsychological performances, gray matter volumes and brain functions. And we also intend to investigate the difference of glymphatic function between CSVD, AD and MCI. Therefore, we would like to apply for the use of ADNI database to obtain data of AD patients and MCI patients, and conduct research with our existing data of CSVD patients.
