Proposed Analysis: We wish to investigate practice effects on neuropsychological tests with the aim of improving measurement of cognitive decline. We have demonstrated in another sample that accounting for practice effects improves MCI diagnosis, and that practice effects can be present even when there is group mean decline on a measure. In that study, the use of practice-corrected data doubled the rate of MCI from 4.5% to 9% and reduced the reversion rate from MCI to cognitively normal (6.1% to 4.8%) (Elman et al. 2018). MCI cases may be missed in the ADNI dataset when practice effects are not considered. If successful, these analyses will improve the goal of ADNI in mapping cognitive decline by more accurately labeling those who demonstrate cognitive decline at follow up. Specifically, we will calculate practice effects across two visits within ADNI by comparing scores of subjects who take the tests for the first time with the scores of age-matched others who are taking the tests for a second time. We hypothesize that the use of practice-corrected data improves prediction of MCI and progression to dementia. Additionally, we hypothesize that the use of practice-corrected cognitive scores will change associations between biomarkers and diagnosis group (i.e., cognitively normal, MCI, dementia) because not accounting for practice effects will obscure cognitive decline in some subjects.
