Proposed Analysis: We aim to emulate a target placebo-controlled trial to estimate 4-year effect of antihypertensive drug use on prevention of cognitive decline and dementia among individuals free of dementia at baseline, compared to those who had no use of antihypertensive drug. The eligible participants include those who are 65 years or older at baseline and with SBP>=140 mmHg. We will use new-user design---for those who use antihypertensive medication at screening will be excluded, and those who use antihypertensive medication at baseline will be assigned to intervention group, and those who do not use antihypertensive medication at baseline will be assigned to placebo group. Cox models will be used to estimate the effect of antihypertensive medication on preventing incident dementia, and linear mix-effect models will be used to estimate the effect of antihypertensive medication on preventing cognitive decline.
