Proposed Analysis: Dear Professor, I am a researcher in Xuanwu Hospital of Capital Medical University. My supervisor is Professor Ying Han. Her research direction is Alzheimer's disease (AD), more specifically, is subjective cognitive decline (SCD). Our team has established the Sino Longitudinal Study on Cognitive Decline (SILCODE) project in China. In the SILCODE cohort, the enrolled subjects underwent detailed neurological function examination, Multi-parameter magnetic resonance imaging (MRI) examination, FDG-PET and amyloid-PET (AV45) examination, and some subjects were collected blood and urine samples. The biological samples were completely stored in the clinical sample database of Xuanwu Hospital. The protocol of SILCODE project can be acquired in the PMID: 31350244. I mainly explored the early diagnosis of AD. By using radiomics analysis method, our group found that the radiomics features of multiparametric MRI scans could represent potential biomarkers of cognitively normal individuals who in the Alzheimer's continuum (PMID: 33344457). In addition, our group proved that the hypometabolism of right middle temporal gyrus could be a typical feature of SCD, and the large-scale hypometabolism in patients with symptomatic stages of AD may start from the RMTG, which gradually progresses starting in the preclinical stage (PMID: 33827675). Recently, our group used the voxel-based scaled subprofile model/principal component analysis (SSM/PCA) method to generate a preclinical AD-related metabolic covariance pattern, and suggested that the pattern showed a certain ability to discriminate Abeta+ NCs from Abeta- NCs (under review). In addition, our team has also explored the Abeta protein in blood (PMID: 33510631), and I am also doing research on brain-derived exosomes in blood. However, the subjects in our cohort did not have cerebrospinal fluid samples, and we did not have tau-PET data. In addition, the SILCODE cohort is mainly a cross-sectional study, at present, the follow-up time of the enrolled subjects is relatively short, and there are very few clinical converters, which greatly limits our researches. And notably, previous reports showed that the reduction in glucose metabolism in AD-sensitive areas is not directly related to Abeta deposition, and other evidences suggested that the elevated brain Abeta deposition alone is probably insufficient to produce neuronal damage and cognitive changes; the correlation between Abeta and metabolism or cognition is likely to be mediated by neurofibrillary tangles with a temporal delay. Furthermore, we also hope to have an external verification dataset to further verify our results. I hope to get your approval to allow me to use the ADNI, ADNIDOD and AIBL data. If approved, I will strictly abide by the regulations. Firstly, I will verify my previous research results in these datasets. Secondly, I hope to explore the differences of SCD between Chinese and other ethnic groups from the perspective of imaging and biomarkers. As we know, the SCD is seriously influenced by cultural backgroud. Thirdly, I hope to analyze the follow-up data in the database and use the survival analysis model to explore the risk factors of clinical deterioration. I hope these factors include high-latitude imaging features (radiomics features) and multi-parameter imaging features. Fourthly, I want to explore the effects of Abeta and tau on multi-parameter MRI according to different diagnostic classifications (according the NIA-AA 2018 diagnostic profile).
