Proposed Analysis: Mild cognitive impairment (MCI) in isolated REM sleep behavior disorder (iRBD) is a risk factor for the subsequent neurodegeneration, in particular dementia with Lewy body and Parkinson’s disease dementia. Among various neuroimaging biomarkers, FDG-PET has been frequently used to ascertain the presence of Lewy body disease even at early symptomatic stages. However, very little work has been done on the association between brain metabolic changes and cognitive impairment in iRBD. We aim to identify brain metabolism pattern related to cognitive decline in iRBD and the neuroimaging marker’s predictive value for future disease conversion. To this aim, we are planning to extract brain hypometabolism maps at the single-subject level. Our iRBD cohort have followed up an average 4 years, and about 30% of patients converted Lewy body disease. We expect that the single-level metabolism maps allow to predict future cognitive decline and disease conversion more precisely at early point. The vast of ADNI data will be use as normal controls to define individual hypometabolism map of our iRBD patients.
